Mandatory Travel Advisory Form.

Guests arriving into LAX from other countries or states are required to complete a mandatory form acknowledging that they have read and understood California’s travel advisory. Travelers over the age of 16 must submit this form online prior to or upon arrival at Los Angeles International Airport, Van Nuys Airport, or Union Station. Failure to submit the form is punishable by a fine of up to $500.

Some cities and counties have different guidelines and you should check with the health department at your destination for their specific rules. As of December 17, 2020, the Los Angeles County Department of Public Health advises that travelers entering LA County should self-quarantine for 10 days after arrival, but advises that individuals should still monitor their health for 14 days.
